Member resources

Everything for members, in one place.

Your ID card, your benefits, your claims, and the forms you might need. Log in to see your own plan details, or use these pages to understand how each piece works before you sign in.

Member tools

Start with what you came for.

Each page explains what the tool does, what you can do without logging in, and who to call if you would rather have a person handle it.

01ID card

Get my ID card

Your ID card is what a front desk or pharmacy needs to bill your plan. See what is on it, how to get a replacement, and what to do at an appointment before your card arrives.

02Benefits

Check my benefits

Find out what your plan covers, what you pay, and where you are against your deductible and out-of-pocket maximum, plus how to read a benefits summary line by line.

03Claims

Search claims

See how a claim moves from your visit to your explanation of benefits, what each status means, and what to do when a claim is denied or a bill looks wrong.

04Forms

Forms library

The forms members ask for most, grouped by what they do: enrollment changes, reimbursement, pharmacy, appeals, and privacy authorizations.

05Mobile

Get the mobile app

Carry a digital ID card, look up benefits, and check claims from your phone. See what the app does and how to set it up on a new device.

Without an account

You are never locked out of your own coverage.

The member services number printed on your ID card reaches the team that administers your specific plan. They can take care of anything below, and they can do it whether or not the online portal is open for your plan yet.

  • Request a replacement ID card by mail or email
  • Ask a representative to read your benefits and current deductible to you
  • Check whether a claim has been received, processed, or paid
  • Have any member form sent to you
  • Update your mailing address, phone number, or language preference
  • Start an appeal or grievance about a coverage decision

Good to know

Questions members ask first.

Do I need an online account to use my coverage?
No. Your coverage works the same whether or not you ever log in. An account is a faster way to look things up yourself, but every request can be handled by calling the member services number printed on your ID card.
Who can see my benefits and claims?
Only you, and anyone you have authorized in writing. Access is checked on our servers against your plan record every time, so a shared link or a saved page never shows another person their information. To let a family member or caregiver act for you, ask member services for an authorized representative form.
My plan is through my employer. Is this the right place?
Usually yes for benefits, claims, and ID cards, because those come from the plan itself. Anything about who is eligible, when you can enroll, or what your employer pays goes through your employer benefits team instead, since they administer that part.
I have Medicare. Does anything work differently?
The tools are the same, but Medicare rules are not. Enrollment periods, drug coverage stages, and appeal timelines follow Medicare requirements, and those are described in the plan documents you were sent. For questions about original Medicare or your Medicare number, you can also call 1-800-MEDICARE, 24 hours a day, 7 days a week.
What if I am no longer covered?
You can still ask for claims and documents from the period when you were covered, since those records belong to you. If you need new coverage, losing a plan is a qualifying life event, which usually opens a 60 day window to enroll outside of Open Enrollment.
