Plan disclosures

What you should know about us.

Before you compare plans or talk to an agent, here is exactly what My Live Health is, what it is not, how we get paid, and what our quotes do and do not promise.

The short version

Four statements, in plain language.

We are

A licensed insurance agency

My Live Health LLC sells insurance on behalf of the insurance companies whose products we are appointed to offer. Our agents are licensed in the states where they transact business.

We are not

An insurance company

We do not underwrite policies, set premiums, build networks, decide claims, or administer benefits. The insurance company you choose does all of that under its own contract with you.

We are not

A government website

This is a commercial website. We are not affiliated with, endorsed by, or acting on behalf of the federal Health Insurance Marketplace, HealthCare.gov, Medicare, Medicaid, the Centers for Medicare and Medicaid Services, or any state agency.

We are not

A source of medical or legal advice

Everything here is general information to help you compare coverage. It is not medical advice, tax advice, or legal advice, and it does not replace your plan documents.

How the agency model works

An insurance agency stands between you and the insurance companies. We are appointed by carriers to present and sell their products, we help you compare what is available where you live, and we submit your application. From that point on, your relationship is with the insurance company: it issues the policy, bills you, builds the provider network, and pays or denies claims under the terms of its own documents.

This matters most when something goes wrong. If a claim is denied or a service is not covered, we cannot overturn that decision. We can explain it and help you find the right process, which is described on the grievance and appeal page.

The plans we offer

We do not offer every plan available in your area. What we can show you is limited to the plans from the carriers we are appointed with in your state, and that list changes as appointments and product lines change. Other plans may exist that we cannot quote.

For Medicare specifically: we do not offer every plan available in your area. Any information we provide is limited to the plans we do offer in your area. To get information on all of your options, contact Medicare.gov or call 1-800-MEDICARE, 24 hours a day, 7 days a week. TTY users can call 1-877-486-2048.

For individual and family coverage under the Affordable Care Act: you can always compare and enroll in Marketplace plans yourself, at no cost, through HealthCare.gov or the marketplace your state runs. Using an agent does not change the premium you pay.

For Medicaid and CHIP: only your state Medicaid agency can determine whether you qualify and act on an application. We can explain the program and help you understand your other options, but we do not make eligibility decisions. See the Medicaid and CHIP page.

What a quote is and is not

Quotes on this site are estimates generated from the information you enter. Premiums, benefits, cost sharing, provider networks, and drug lists vary by ZIP code and county, by plan year, and by the details of your household, and they can change when a carrier files new rates. Any subsidy amount shown is an estimate; the final amount is determined by the Marketplace based on the information in your application and reconciled on your federal tax return.

A quote is not an offer of coverage and not a guarantee of acceptance or price. Coverage exists only after the insurance company approves your application and issues a policy, and only on the terms in that policy. Where anything on this site conflicts with your plan documents, your plan documents control.

How we are paid

When you enroll in a plan through us, the insurance company pays My Live Health a commission. You do not pay us a separate fee, and enrolling through an agent does not raise your premium: rates for a given plan are filed by the carrier and are the same whether you enroll through us, directly with the carrier, or through a government marketplace.

Commission amounts differ by carrier and product. We tell you this so you can weigh our recommendations with it in mind, and so you can ask. If you want to know how we are compensated on a specific plan you are considering, ask your agent or email contact@mylivehealth.com and we will tell you.

Licensing

My Live Health LLC and its agents hold the insurance producer licenses required in the states where they transact business. Licensing status varies by state and by product line and changes over time, so this page does not list license numbers that could become stale. For current licensing information for your state, including the license number under which an agent is acting, contact us at contact@mylivehealth.com or (833) 741-1902, and ask the agent you are speaking with. You can also verify a producer license with your state department of insurance.

Referrals to partner agencies

Your inquiry may be referred to a licensed insurance agency partner when that agency is better placed to help with your state or the coverage you asked about. In that case the partner agency, not My Live Health, works with you and receives the commission on any plan you buy through it. The partner may contact you by phone or email about coverage. How that referral is handled is described in the privacy center.

Not every product is major medical coverage

Some products we can quote are designed to sit alongside a health plan rather than replace one. Dental, vision, accident, critical illness, hospital indemnity, and similar supplemental products pay limited, defined benefits and do not qualify as minimum essential coverage. Short term medical plans, where they are available, are not required to cover pre-existing conditions or the essential health benefits and can decline applicants based on health history.

Before you buy anything other than a major medical plan, make sure you understand what it does not cover. Your agent is required to explain that, and the plan documents state it in full.

Third party content and rate information

Plan data, provider directories, and drug lists shown on this site come from insurance companies and other third parties. We work to keep them current, but we do not control them and cannot guarantee they are accurate or complete at any given moment. Always confirm with the carrier that a specific doctor is in network and a specific drug is covered before you enroll or receive care.
